Kit of Care

Patients bounce back to hospital after discharge often because they are not able to maintain the regiments of treatment at home. There are multiple reasons for this non-adherence such as lack of education or care-giver support, insufficient training to transition, not following the care plan, lack of ability to problem solving etc. To solve these problems, we have developed a 'Kit of Care' which will help the patient and the caregiver to maintain continued healing at home. The anticipated impact of 'Kit of Care' is to assist in reducing hospital readmission.

Interactive Pre-packaging Helps Medication Management

Brief: The number of prescriptions and amount of patient education affect medication adherence, which can lead to a higher probability for readmission. Our pre-packaging solution, which includes an interactive QR code, gives the patient an opportunity to be educated and reduce medication confusion.

Behavior change in hospital: Sodium restricted diet
Congestive heart failure is a chronic condition and patients are recommended to comply with dietary sodium restrictions. However research indicates that most patients find it difficult to follow these restrictions and may need help in understanding what constitutes a sodium restricted diet. Through our smart tray we propose a concept that would serve as an interactive tool to provide education and create awareness about a sodium restricted diet while eating their meals in the hospital.

Reducing the Impact of Healthcare-Acquired Infections on Hospital Readmissions
Poor hand sanitation is one of the key factors leading to increased healthcare-acquired infections (HAIs), ultimately leading to a higher risk of readmission. Non-compliance rates with hand hygiene are high (close to 50%)in hospitals, and even higher in post-discharge facilities, such as Long Term Care Facilities (LTCFs), and up to 3.8 million infections a year occur in LTCFs. We have designed a motion-dependent hand-sanitizer to be used to spray alcohol-based antiseptic on door handles, to sanitize the hands of healthcare personnel and patients with a focus on LTCFs and post-discharge facilities.
