Patients bounce back to hospital after discharge often because they are not able to maintain the regiments of treatment at home. There are multiple reasons for this non-adherence such as lack of education or care-giver support, insufficient training to transition, not following the care plan, lack of ability to problem solving etc. To solve these problems, we have developed a 'Kit of Care' which will help the patient and the caregiver to maintain continued healing at home. The anticipated impact of 'Kit of Care' is to assist in reducing hospital readmission.

'''Brief:''' The number of prescriptions and amount of patient education affect medication adherence, which can lead to a higher probability for readmission. Our pre-packaging solution, which includes an interactive QR code, gives the patient an opportunity to be educated and reduce medication confusion.

Poor hand sanitation is one of the key factors leading to increased hospital-acquired infections (HAIs), ultimately leading to a higher risk of readmission. Although hand-washing stations and sanitizers are commonly available in the hospitals, non-compliance rates are high and more measures need to be taken. We have designed a motion-dependent hand-sanitizer to be used to spray alcohol-based antiseptic on hospital door handles, to sanitize the hands of healthcare personnel as they enter ICU and surgical patient wards.

Introduction

The main objective of this interdisciplinary course HS8803/COA8823/CS8803 (part of the Healthcare Design of the Future Series) is to to reduce hospital readmissions by design -

The course is mainly intended for the students to

1. To experience and learn techniques for successful interdisciplinary design

2. To be exposed to the unique challenges of healthcare design

3. To learn the importance of environment in the healing process

4. To understand the roles and skills of multiple disciplines in the design process including: the architect, the systems engineer, computer/technology engineer, and the clinician

To have a track of who is editing what ? all students should create a new account before accessing this wiki. You can do so by clicking on the create an account or log in link at the top-right of every page. When you make edits, please use the "preview" feature to make sure your changes are final, and then don't forget to save. Use the "edit summary" to describe what you did and why. One should ensure that their additions reflect a positive collaboration in the interest of the course.

Writing for Wikipedia articles is a bit different from writing on a standard word processor. Instead of a strict "what you see is what you get" approach, wiki uses simple text codes for formatting. The approach is similar to that used in writing HTML for web pages, but the codes are simpler. The MediaWiki help is a great place to start learning about all the features available.

For the new users, please read the editing guidelines of Wikipedia for the guidelines regarding the Style and the Contents, Heading and subheadings and topics on how to handle the links and images to our wiki.
