Problem Statement
Patient Navigators fulfill a valuable function in the Emergency Department, assisting patients and clinicians and improving follow up care following patient release. However, Navigator programs are often resource-limited due to the difficulty in obtaining reimbursement for such services. We can increase the number of patients served by Navigators if we can increase their productivity and efficiency, and we can increase the Navigators’ effectiveness.
Purpose
Proposed Solution
Formalize intake function
- Collection of patient data
- Handover from triage nurse or ED physician to Navigator
- Automate repetitive tasks
Scheduling appointments
- Follow up with patients
Centralize resource data
- Physician practices
- Outpatient clinics
- Social services
